1. Click on the dropdown menu at the top right of the screen and select “Manage Branch”
2. Select “Fillable Form Templates” from the panel on the left
3. Click on the “Add New Template” button at the top right

1. After entering the details for the fillable form template, click on “Save Template” at the bottom right
2. To edit a created template, select the “Edit” option from the right column of the fillable form template panel
3. Be sure to click “Save Template” at the bottom right to confirm and save the updates to the edited fillable form template
